31+ days past due renewal penalty fee: $25

Get started here: In order to apply a candidate must meet the following 2 requirments

1) Successfully complete an NAPT approved Phlebotomy training course.

2) Candidate must have a High School Diploma or GED to qualify.

Application must be submitted within 10 days of test date.
